Here at Blackburn with Darwen we are immensely proud of the work we do and have an exciting opportunity for two social workers to join our growing team and would welcome applications from newly qualified social workers, experienced social workers or Approved Mental Health Social Workers.

You will be based in the Community Mental Health Team based at Daisyfield Mill and work closely with both health and social care staff. In this team we support individuals with both lower end of mental illness (primary care neighbourhood post) and the serious and enduring mental health conditions, complex care & support needs, including support to families and carers with the aim to improve mental health and wellbeing and to promote recovery and social inclusion.

Two posts:

Hospital Discharge Social Worker

We pride ourselves on the relationships we have with our health partners and an opportunity has arisen for a hospital social worker to join our small hospital team who work with people who find themselves in hospital following a period of mental health deterioration. Your job would be to support them to leave hospital and reconnect with their home and community. The role involves working with all the hospitals across Lancashire where are patients are admitted.

Primary Care Neighbourhood Social Worker

This post involves working closely with your peer PCN Social Workers and the community at that primary level working with people who historically may have fallen between the gaps and need some short-term support due to their circumstances. Key task of this post is relationship building with the service users and professionals from a host of agencies from a strengths-based perspective enabling and empowering people to take back control of their lives.

What do we need from you?

Genuine interest and passion in working with people from a strengths-based approach with a willingness to support people’s independence, resilience and ability to make informed choices for themselves about their mental health and well-being.

Relevant mental health and social care experience. (We will offer posts to less experienced social workers with the right skills at interview).

Ability to work autonomously and as part of a multi-disciplinary team ensuring the best outcomes for residents of Blackburn with Darwen.

We are keen to attract AMHPs and BIAs and recognise the specialist skills, expertise and legal knowledge you bring to the team.

You will have experience of working with people of varying care & support needs and be able to implement the Care Act 2014, completing high quality assessments and care and support plans.

You will manage a realistic caseload using job planning and be able to prioritise your work dependent on need and risk and at all times be flexible and responsive to the people you support and your colleagues.

You will need to be registered as a Social Worker with Social Work England (or be close to if a final year social work student).

You will also need a clean driving licence and access to a car. 

Salary will be negotiable based on skills and experience.
